Resurrection Health Care Pre-Registration
Resurrection Health Care offers you two convenient ways to pre-register for your scheduled appointment for Medical Imaging or Maternity services at the hospitals listed below.
After you schedule your appointment for your medical imaging test or prior to utilizing Maternity services, please complete the pre-registration form. The form must be completed online or faxed at least 48 hours prior to your appointment to allow time for processing. If you do not provide all of the requested information, you may be contacted by telephone.
